Create and edit a form template

Last updated: November 4, 2026

A form template is a reusable set of questions, such as an inspection checklist or a job completion report. You add forms to jobs from your templates.

Create a form template

  1. Select Company settings > Forms > New form.
  2. For the first question, enter a title and choose a question type.
  3. For a choice question, enter the options. Select Add new option to add more.
  4. Add helper text or mark the question as required, if needed.
  5. Select Add another item to add more questions.
  6. Enter a Form name. A Form description is optional.
  7. Select Create form.

Question types

Question type What people enter
Short response A short text answer
Long response A longer text answer, with line breaks
Single choice One option from a list
Multiple choice One or more options from a list
Date selection A date
Upload file One or more files

Edit a form template

  1. Select Company settings > Forms.
  2. Select the template’s title.
  3. Change question titles, types, options, or settings, or select Add another item to add questions.
  4. Select Save.

Note: If you leave without saving, your changes are lost.

Duplicate, reorder, or remove a question

  • Duplicate: Create a copy of a question, including its title, type, helper text, and settings. Rename the copy, then select Save.
  • Reorder: Drag a question to a new position. The new order saves automatically.
  • Remove: Delete the question, then select Save.

How template changes affect existing forms

Changes to a template apply only to forms you add afterward. Forms already added to jobs keep their questions, options, and settings, whether they’re incomplete or completed.
